Providing Hospital-Level Care in the Comfort of Home

Covenant Health now offers in-home hospital services, virtual care and remote monitoring for around-the-clock hospital care for patients who are eligible. In partnership with Medically Home Group, Inc., Covenant Health can provide hospital-level care to patients in their homes.

What is Advanced Care at Home?

With Advanced Care at Home, Covenant Health physicians and nurses can monitor patients in their homes from a clinically integrated, virtual care command center that operates 24 hours a day. In addition to remote care that leverages the technology and infrastructure at Covenant Health’s operations center, in-person services will be delivered via home visits from qualified medical professionals.

Eligible Conditions

The program is designed for patients who require high-acuity care. Conditions in which patients are eligible for this program may include:

  • Heart Failure
  • Chronic obstructive pulmonary disease (COPD)
  • Asthma
  • Pneumonia
  • Cellulitis/Soft Tissue Infections
  • Complicated UTI/ Pyelonephritis
  • Gastroenteritis/Dehydration

Eligible Services

Patients will receive hospital-level medical support services as needed. Those services may include:

  • Medication management
  • Laboratory services
  • IV treatments
  • Rehabilitation therapies (physical, speech and occupational)
  • Oxygen treatments
  • X-ray and ultrasound

Advanced Care at Home Individualized Care Plan

  • 24 hours-a-day, 7 days-a-week access to your care team who will respond to your medical needs via video and telephone
  • Vital signs monitoring using a telehealth platform
  • Video visits by your care team
  • In-home visits by health care professionals
  • Lab tests, mobile imaging like x-rays and ultrasound, and IV therapies performed in your home
  • Collaboration with your primary care physician and/or specialists
  • Complete report of your care sent to your primary care physician
