How does the Connected Care Management (CCM) program work?

CCM services are in addition to the regular medical care you receive from our office and are specific to the management of your chronic health conditions.

You’ll receive continuous care through access to our Care Team. The Care Team can provide both and urgent appointments so that you get as much support as you need.

CCM services can help reduce office visits because the program is conducted primarily through telephone, portal messaging, or video conferencing.

These services may help avoid the need for more costly medical care in the future by proactively managing your health rather than focusing only on treating severe and/or acute illnesses. CCM is about treating the whole person!

How do I receive CCM services?

You’ll be assigned to a designated member of our Care Team, either an RN Care Coordinator or LPN Care Navigator.

Your nurse will be able to:

• Assess your healthcare needs.
• Review and manage your care plan.
• Ensure you receive timely and comprehensive care.
• Connect you with additional services such as referrals to specialists, lab work, or diagnostic testing.

These services will be conducted through monthly visits via telephone, text or portal messaging. With your written permission, your caregiver or a family member may also communicate with your nurse.

Your Care Team will work closely with you and your primary care provider to create the best possible care plan for your chronic conditions and overall health, one that also works with your lifestyle.  A written or electronic copy of your care plan is available.

What do I need to do to succeed with CCM?

To achieve your healthcare goals that are described in your care plan, you’ll need to:

• Keep all regularly scheduled appointments.
• Take medications as prescribed.
• Follow your care plan.
• Contact us with any questions or concerns
• Notify us any time you visit the ER, or are discharged from a hospital or skilled nursing facility.

Cost of Services

CCM services may be free or only require a small copay if covered by your healthcare insurance. If you’re paying out of pocket, you’ll get a bill for services received. If you have any questions regarding insurance or billing, please let your Care Team know.

Connected Care means you’ll get personalized services whenever you need them from a Care Team who knows about you and your conditions.
